Introduction: The semi-direct composite resin restorative technique is na excellent treatment option for restorations in posterior teeth with extensive loss of crown structure. The semi-direct technique can be conducted both intraorally and extraorally. In the extraoral semi-direct approach, the restoration is fabricated on top of a rigid or semi-rigid model of the previously prepared tooth, eliminating the need of referring the piece to a prosthetic laboratory and thereby reducing the costs and treatment time of the procedure. Objective: To report, in the form of a clinical case, the fabrication of an overlay restoration using the semi-direct technique on tooth #26, which had extensive coronal structure loss. Case report: Patient E.T.F.P .M., 31 years old, complained of extensive loss of coronary structure following the fracture of an amalgam restoration. The extraoral semi-direct technique was chosen. Tooth preparation was performed using diamond burs, followed by molding through impression-taking with alginate and, after, by fabricating the plaster model. The composite resin restoration was created on the model using A3D (Empress Direct, Ivoclar) and A1E (Empress Direct, Ivoclar) resins. A GrandValo (Ultradent, Salt Lake City, Utah, USA) curing light unit with an intensity of 1200 mW/cm² was used for photoactivation. The restoration was cemented using dual-cure resin cement in shade A2 (Allcem, FGM, Joinville, Santa Catarina, Brazil). Occlusal adjustments, finishing, and polishing were carried out with fine-grit diamond burs, rubber polishers, and abrasive discs. Conclusions: The extraoral semi-direct technique is a viable and cost-effective option for restoring teeth with extensive coronal structure loss, in spite of requiring increased clinical time on the part of the dentist.
