Introduction: With the advent of the pandemic, several impacts on the process of thinking, doing and promoting health. In this scenario, several assistance programs established in Primary Care (AB) had their activities suspended, including HiperDia, thus breaking the integrity and longitudinality of the care process. Therefore, there is a need to return to activities in a comprehensive, multidisciplinary and progressive way. Objective: To present the experiences, perceptions and inferences of the team of residents during the reintegration process of the HiperDia program during the Covid-19 Pandemic. Methodology: This work is a descriptive experience report with a qualitative approach on an indefinite interventionist activity carried out by a multidisciplinary team in the city of Itabuna/BA. Results and discussion: With the return of activities, awareness of the team and the active search of users, it was possible to promote care activities, health promotion and disease prevention. In this context, two interventionist activities were implemented: The Mutirão do HiperDia and the Multiprofessional Group. During these consultations and follow-ups, it was possible to identify and mitigate multiprofessional stress factors that culminated in increased blood pressure and blood glucose levels. Conclusion: Therefore, we conclude the imperative of a progressive, integral and multidisciplinary return to care, as well as the need to insert education and health activities and intra-territorial actions to improve the biopsychosocial quality of life.
