Introducción: Adolescents face specific challenges regarding condom use, which may increase risks to their sexual and reproductive health. Identifying barriers and determinants is crucial for effective preventive strategies. Objetivo: To analyze the barriers and determinants of condom use among adolescents. Metodología: Systematic review registered in PROSPERO (ID: CRD42024500956). Searches were conducted in PubMed/Medline, Web of Science, Scopus, LILACS, and SciELO databases. Observational studies published between 2018 and 2023, involving adolescents aged 13–18 years, were included. Resultados: A total of 4238 studies were identified, of which 26 met the inclusion criteria. These studies covered 31 countries and approximately 519,020 participants, with a mean age of 14.36 years. Condom use was associated with age, marital status, and the use of long-acting contraceptive methods. Main barriers included social stigma, lack of sexual education, communication difficulties with parents and partners, moral/religious issues, limited accessibility, long-term relationships, and concerns about sexual performance. A decline in condom use was observed with increasing age and in monogamous relationships; condom use was also lower among adolescents with multiple partners or using long-acting contraceptive methods. Conclusiones: The findings may inform the development of targeted health education strategies for adolescents, addressing the identified determinants to promote condom use.
