Methodology: This is an exploratory, descriptive study. To this end, secondary data from module II of the PMAQ-AB were used on the work process of the Primary Care teams specifically from the northeast region of Brazil. Results: This is an exploratory and descriptive study. For this purpose, secondary data from the PMAQ-AB module II are used on the work process of the Primary Care teams specifically from the Northeast of Brazil. Conclusions: It was concluded that there is a need for greater attention to actions taken with patients with chronic diseases in the region studied, as it was found that the workforce, when stimulated and encouraged, can change the local reality and, consequently, the health indicators.
